For security reasons, OutSmart uses several pieces of information to secure a patient's medical information. One of these pieces of information is the patient's e-mail address.
In the case of a child or other dependent, you might want to use your own e-mail address for all communications with your child's health care practitioner.
This is ok, except that you yourself might also be seeing a health care practitioner that uses OutSmart as their medical records system, and you might want to use the same e-mail address for your records as well.
Since every e-mail address is used to identify a single individual in OutSmart, if you want to use your own e-mail address for your child or dependent's medical records, you must modify it as follows:
If your address is healthymom@outsmartemr.com, and you are filling this form out on behalf of your son Joe, then use the e-mail address healthymom+joe@outsmartemr.com.
Use this e-mail address whenever you are interacting with OutSmart Medical Records on behalf of your child or dependent. This includes the Patient Portal.
The '+joe' portion of the e-mail address helps us to identify the child's records as separate from your own, but any e-mail
communications sent to that address will still end up in your inbox.
