Once a patient is logged into their Patient Portal, they can view, complete and submit any forms sent to them by their practitioner.
Step 1: Completing Forms.
- Login to Patient Portal
- Select Forms.
Forms not completed will be labeled “Not Started” in blue.
- Select the “Fill Out And Submit” pen icon under “Actions“
- Fill out all Required Questions with a red star.
- Select Submit (blue box), in the top left.
Once form is submitted, a page will open verifying it was successful.
